Next Generation NCLEX question types
How to answer matrix, bow-tie, cloze, highlight, and select-all items, and how they are scored.
NCLEX-RN and NCLEX-PN include Next Generation (NGN) item types. The NP exams use multiple choice only. Each item states its task above the answers. Read it carefully, because it tells you exactly what you are being asked.
- Select all that apply — tick every correct option (square checkboxes). Scored +1 for each correct pick and −1 for each wrong one, never below zero.
- Matrix / grid — pick one answer per row. Each row is worth a point.
- Cloze (drop-down) — choose a word or phrase from each drop-down in the passage. Each blank is worth a point.
- Highlight — tap the phrases in the passage that answer the task. Scored like select-all.
- Bow-tie — fill five boxes: the condition in the middle, two actions to take, and two parameters to monitor. Drag options in, or tap an option to drop it into the next empty box. Each correct box is a point, with no deductions.
Case studies
A case study follows one patient across six linked steps, the way the NCSBN Clinical Judgment model is tested. The client record sits beside each question in tabs. Answer, tap "Submit", then "Next step", until you can "Finish case". Case studies need the All-Access Pass.
Filter Study Mode by item type ("Next Generation only", or a single format) to drill the formats you find hardest.
