Built to the 2026 NCSBN test plan

Every question is tagged to one of the eight client-needs categories, and your analytics report by that same breakdown — so a weak area maps straight onto the blueprint.

Client-needs categories

Official percentage ranges from the 2026 test plan.

  • Management of Care15–21%
  • Safety and Infection Prevention and Control10–16%
  • Health Promotion and Maintenance6–12%
  • Psychosocial Integrity6–12%
  • Basic Care and Comfort6–12%
  • Pharmacological and Parenteral Therapies13–19%
  • Reduction of Risk Potential9–15%
  • Physiological Adaptation11–17%

How the exam runs

National Council of State Boards of Nursing.

  • Computerized adaptive (CAT) — 85 to 150 items, 15 of them unscored pretest
  • 5 hours total, including all breaks
  • Clinical judgment measured by 18 case-study items plus ~10% stand-alone items
  • 2026 test plan in effect April 1, 2026 through March 31, 2029

We don't fake the adaptive algorithm. The real NCLEX is computerized adaptive testing, which only NCSBN can run. Our practice exams are fixed-length and timed — they build the same knowledge, reasoning, and stamina, and report by client-needs category.

NCSBN sets the passing standard on a logit scale, so there is no official percent-correct pass mark. Percentages in our app are a study signal, never a predicted NCLEX result.

Next Generation NCLEX

Practice every Next Gen item type — not just multiple choice

NGN measures clinical judgment with item types that look nothing like the A/B/C/D questions most banks still ship. You get all of them here, built and graded the way NCSBN presents them — matrix grids, drop-downs inside the sentence, highlighting a nurses' note, the bow-tie diagram, and unfolding case studies with a tabbed client record. Test day should be the second time you see these, not the first.

Multiple response

Select all that apply

Square controls, more than one key, and credit for the ones you get right — the scoring that replaced all-or-nothing SATA.

Scoring: +1 per correct selection, −1 per incorrect, floored at zero — 3 of 4 right never scores like none.

Matrix / grid

One judgement per finding

Findings down the side, categories across the top, one pick per row — the item type that shows whether you can sort a whole assessment, not just spot a single answer.

Scoring: 0 or 1 per row, so every finding you classify correctly counts on its own.

Cloze

Drop-downs inside the sentence

You complete the clinical statement itself, choosing from a list at each blank — so the reasoning has to hold together across the whole sentence, not just at one keyword.

Scoring: 0 or 1 per blank, each graded on its own.

Highlight

Find it in the nurses’ note

No option list at all — you click the findings that matter inside the chart, exactly as the exam presents it. Noticing what is abnormal is half the item.

Scoring: +1 per correct phrase, −1 per incorrect, floored at zero.

Bow-tie

A whole clinical judgement in one diagram

Name the condition, then the two actions it demands and the two parameters that tell you they worked. Drag on a desktop, tap on a phone — five boxes, one line of reasoning.

Scoring: 0 or 1 per placement, with no deduction for a box you get wrong.

Unfolding case study

Six items, one client, a chart that keeps changing

The case study is where most of the clinical-judgment score comes from: one client, six linked items, and a tabbed record that updates as the situation evolves. We put the record beside the item — the way the exam does — instead of stacking panels above it.

Scoring: Each of the six items is scored on its own, by its own format's rule.

Every sample above is a teaching item written for this page, not a real NCLEX question — NCSBN's items are secure and we never reproduce them. Your question bank is written to these same formats and reviewed by a board-certified clinician before it is served.

Taking the NCLEX-PN?

We have a separate NCLEX-PN bank, written to the PN test plan and to practical-nurse scope — because relabelling RN questions would teach the wrong practice boundaries.

FAQ

NCLEX-RN Prep FAQs

Do your practice exams work like the real adaptive NCLEX?

No, and we would rather say so plainly. The real NCLEX-RN is computerized adaptive testing (CAT) — it picks each next item from how you answered the last one and ends anywhere between 85 and 150 items. We give you fixed-length timed practice forms instead. They build the same knowledge and stamina and they report by client-needs category, but they do not reproduce the adaptive algorithm, and no prep company outside NCSBN can.

What score do I need to pass?

There is no official percent-correct pass mark. NCSBN sets the standard on a logit scale (0.00 logits for the RN exam, effective through March 2029), so a raw percentage simply is not how the real exam is scored. Any percentage you see in our app is our own study convention for tracking progress — treat it as a practice signal, not a predicted NCLEX result.

Is the question bank built to the current test plan?

Yes. Every question is tagged to one of the eight client-needs categories of the 2026 NCSBN NCLEX-RN Test Plan, which took effect April 1, 2026 and runs through March 31, 2029. Your analytics report by that same breakdown, so weak areas map directly onto the blueprint.

Who writes and checks the questions?

Every item is drafted against standard entry-level RN references, passes an automated accuracy check, and is then reviewed and signed off by a board-certified clinician before any student sees it. Nothing unverified is ever served.

Does the pass auto-renew?

No. It is a one-time payment for the months you choose — 1, 3, 6, or 12. Access simply ends when the window is up, and buying again stacks new months onto whatever you have left.

Do I need a credit card to try it?

No. Create a free account and start practicing immediately — get the All-Access Pass only when you are ready.
